Zusammenfassung
Die Vorteile der minimalinvasiven Eingriffe liegen auf der Hand. Kleinere Schnitte
mit minimiertem Zugangstrauma führen neben der besseren Kosmetik zu weniger Schmerzen
und zu einer geringeren Anzahl an möglichen Wundkomplikationen. Allerdings werden
diese offensichtlichen Vorteile oft nicht evident durch klinische Studien belegt.
Trotzdem werden Routineeingriffe bei Hernien, bei akuter Appendizitis, an der Galle,
sowie bei entsprechenden Erkrankungen der Schilddrüse zunehmend minimalinvasiv durchgeführt.
Im Rahmen dieser Indikationen werden in Deutschland geschätzt stationär pro Jahr ca.
800 000 Eingriffe durchgeführt, was zusammen ungefähr ein Viertel des Arbeitsaufkommens
der Allgemeinchirurgen ausmachen dürfte. Schon alleine diese hohe Zahl zeigt die Bedeutung
für unser Gesundheitssystem. Besonders unter sozioökonomischen Gesichtspunkten müssen
diese vermeintlich „kleineren“ Eingriffe hocheffektiv mit minimaler Komplikationsrate
unter Gewährleistung einer schnellen Rekonvaleszenz durchgeführt werden.
Diese Übersicht zeigt den Stand und die Weiterentwicklungen der minimalinvasiven Operationsverfahren
mit modernster Technik, mit neuem, immer feineren Instrumentarium und Single-Inzisions-Zugangsports.
Aber auch die zunehmende Expertise der minimalinvasiv tätigen Chirurgen bringen hier
auch auf dem Gebiet des „Alltäglichen“ weitere Fortschritte mit einem offensichtlichen
Vorteil für die Patienten.
Summary
The advantages of minimally invasive interventions are obvious. Smaller incisions
with minimized approach trauma result not only in better cosmetics but also in less
pain and a smaller number of possible wounds-healing complications. However, these
apparent advantages are not always effectively confirmed by clinical studies. Even
so, routine interventions for hernias, for acute appendicitis, on the gallbladder
as well as for the corresponding diseases of the thyroid gland are being increasingly
performed using minimally invasive techniques.
In the framework of these indications it is estimated that in Germany each year about
800 000 operations are performed in hospitals, together these account for about a
quarter of the work load in general surgery. This large number alone demonstrates
the significance for our health-care system. From the socio-economic viewpoint in
particular, these supposedly „minor“ interventions should be performed with high efficiency
and minimized complication rates while guaranteeing a rapid convalescence.
The present review illustrates the current status and further developments of minimally
invasive surgical interventions with the most modern technologies and with newer ever
increasingly refined instruments and single incision port systems. However, also the
ever increasing expertise of the surgeons employing minimally invasive methods provides
further advances in the „daily routine“ with obvious benefits for the patients.
